Please complete this form before April 4th, 2025.

Please note that the content of the answers to this questionnaire will enable the Selection Committee to assess the applications according to the following criteria:

• The nature and amount of time devoted to volunteering
• The role played by the volunteer
• The impact of volunteering on the community (inspires others to volunteer; meets a need)

The winner from each category will be in the running for the “2025 Haut-Saint-Laurent Volunteer Award”. The event will take place on May 2nd, 2025, from 4:30 p.m. to 7:00 p.m at LBA hall.

• Limit of three (3) candidates per organization
• Provide at least one way to allow the organizing committee to reach the nominees directly, either by email or by phone.
• Please make sure the name of the nominee is spelled correctly.
